Whole-Genome Searches Are Revealing Secrets of a Childhood Cancer
Two new studies from The Childrens Hospital of Philadelphia advance the search for genetic events that result in neuroblastoma, a puzzling, often-deadly type of childhood cancer.
Originating in the peripheral nervous system, neuroblastoma is the most common solid cancer of early childhood and causes 15 percent of all childhood cancer deaths.
Only two years ago we had very little idea of what causes neuroblastoma, said study leader John M. Maris, M.D., chief of Oncology and director of the Cancer Center at The Childrens Hospital of Philadelphia. Now we have unlocked a lot of the mystery of why neuroblastoma arises in some children and not in others.
In the largest gene study to date in pediatric oncology, Mariss study team performed a genome-wide association study to discover that common variants in the gene BARD1 increase a childs susceptibility to a high-risk form of neuroblastoma.
A second genome-wide study found that a copy number variation (CNV) a missing stretch of DNA along a structurally weak location on chromosome 1 plays an important role in the development of neuroblastoma. Although CNVs have received much attention from genetics researchers over the last several years, this study was the first example of a specific CNV that predisposes people to cancer.
The BARD1 study was published online in Nature Genetics on May 3, while the CNV study appeared in the June 18 issue of Nature. The researchers made use of highly automated gene-analyzing technology at the Center for Applied Genomics at Childrens Hospital, directed by Hakon Hakonarson, M.D., Ph.D., a co-author of both studies. They used specimens collected from around the world through the Childrens Oncology Group.
The BARD1 gene had already attracted attention from oncology researchers because it is associated with the gene
CA1, which was the first discovered familial breast cancer gene. Researchers have suspected that variants in BARD1 also increased the risk of breast cancer, but no one has found compelling evidence of this, said Maris. Instead, surprisingly, our genome-wide association studies found that BARD1 is a susceptibility gene for neuroblastoma, and perhaps other cancers as well.
Maris added that researchers are now working to understand the mechanism by which BARD1 gene variants act on developing nervous system cells to give rise to cancer during fetal or early childhood development.
Mariss second study, spearheaded by Dr. Sharon Diskin, also of The Childrens Hospital of Philadelphia, found that an inherited CNV located at chromosome 1q21.1 is associated with neuroblastoma. The chromosome region contains a large family of genes that are involved in the development of the nervous system, and the CNV they discovered changes how much of one particular gene is made within normal nerve and neuroblastoma cells.
This study, Maris added, opens up a new area for studying the mechanisms of how CNVs may increase the risk of cancer.
The current findings build on 2008 studies by Mariss lab, one identifying the ALK gene as the major gene predisposing patients to the rare familial form of neuroblastoma, and the other identifying a region of chromosome 6 that increases the risk of the nonhereditary form of the disease. The ALK gene discovery has already led to a clinical trial led by Dr. Yael Mosse of The Childrens Hospital of Philadelphia.
As gene studies continue to better define the genetic landscape of neuroblastoma, added Maris, pediatric oncologists can better develop more precise targeted treatments to improve survival and quality of life for children with this complex disease. The Cancer Center at Childrens Hospital has one of the nations largest research and clinical programs in pediatric oncology.
DNA samples for both studies were provided by the Childrens Oncology Group, a multicenter collaborative research organization in which Maris chairs the committee overseeing basic and clinical research in neuroblastoma. A variety of funding sources supported both studies, including the National Institutes of Health, the Alexs Lemonade Stand Foundation, the Evan Dunbar Foundation, the Rally Foundation, the Andrews Army Foundation, the Abramson Family Cancer Research Institute and the Giulio DAngio Endowed Chair.
Maris is also on the faculty of the University of Pennsylvania School of Medicine. Scientists from six other centers in addition to Childrens Hospital and the University of Pennsylvania contributed to the discovery or replication of the findings.

My grandmother recently got breast cancer and had her breasts removed. After the chemotherapy, she started radaiation. I am not sure what week she is in, but she is 60 years old. Lately she has been telling me she has a very sore throat. So sore that she cannot even swallow her saliva. Does anyone have any remedies or something that wil help her feel just a little better? Just enought so she can eat food? She lives in a different country, so medications wont do much good. Thanks.

A 62 year old Singapore man, who was trying to enter the U.S., was detained at customs for several hours for an odd reason - he had no fingerprints! Even more disturbing is that the cause of his fingerprint loss was due to taking a common cancer drug, capecitabine, also known as Xeloda by its trade name.
A side effect of capecitabine is a condition called hand-foot syndrome, which causes severe skin dryness and peeling of the palms of the hands and soles of the feet. It is a very uncomfortable condition that can really disrupt a person's daily life. Imagine trying to type or pick up an object when you hands are so severely dried, blistering and peeling.
Prevention is key when it comes to hand-foot syndrome. Typically, a person would experience redness and tingling prior to skin peeling. But, the fingerprint-less traveller did not experience effects so severe that it interupted his daily life, so no medical intervention occurred. He and his oncologist clearly were unaware that he no longer had fingerprints.
Experts recommend that if you are taking Xeloda and traveling to countries that require fingerprint identification to bring a statement from your doctor stating your medication and its possible side effect of fingerprint loss.

I just need you all to understand how important what you do is! These are only three out of the over 75 people we currently have who need Spirit Jumps. Think of the lives you touch, the smiles you bring, laughter, curiosity, joy, hope and wonder you bring into the lives of those touched by cancer. And not only the person who has been diagnosed but their parents, siblings, husbands and wives. The power of a small gift is beyond any of our comprehension. Do you understand what you are doing and have done? I dont think you do because it is indescribable!

Heyer handled it well and said that he is close to closing on a house in the 39th district (on Summit Street) and that his family home (where five generation of Carroll Gardeners have lived) is three blocks out of the district. Someone is trying to turn this into an issue; I dont think it will go anywhere if his residency is resolved before a certain date. That was really off the wall, John Heyer said good naturedly.
Brad Lander was asked about the City pension fund scandal and its connection with a Park Slope resident and neighbor of Lander whose name I havent confirmed. Lander looked really mad (the only time Ive ever seen him mad) and he called the question He told the person who asked the question to stand up, which he/she did not. He defended his friend as a very ethical person and a good friend whos only mistake was giving accurate information.
Whoa. It was cool to see Lander angry. He handled the question really well and showed fire and passion in defense of his friend.

Its interesting to see these personalities in action. As Ive said, they display different styles and leadership potential. All seem to agree, for the most part, on issues and core values. But whos the one who will be the best City Council member? Too soon to call but here are my thoughts-in-progress about these men. Ill start with Gary.
Gary Reilly: As the new kid on the block, his local politics and community activism are fueled by a real passion for his newfound home and the ways it could be more livable. In answer to a question about future goals beyond the council he said he wouldnt have the stomach for a city-wide election for mayor, but he needs to have more stomach for this campaign and come on stronger.
Unasked for advice: Lose the deceptively low key style and come on strong about your wonky passion for public transportation, infrastructure and livable streets.
David Pechefsky: I call him the insider/provocateur because hes got ten years of experience working in the central office of the NYC City Council under his belt but he he also understands intimately whats wrong with the system and thinks he knows how to fix it. Hes smart, provocative and good at framing the argument for reform. He comes across like a great thinker/teacher who has a lot to bring to the table in terms of thoughtful ideas and progressive values.
Unasked for advice: Give the voters a keener sense of small picture solutions and demonstrate your leadership qualities and your passion for local government.
Josh Skaller: Hes garnering quite a few impressive endorsements and has the support of those who actively opposed the Atlantic Yards (Daniel Goldstein, State Senator Eric Adams, Norman Sigal, Major Owens, and others). Hes obviously working hard on focusing his message: his belief that citizens can govern.
Unasked for advice: A few more detailed ideas would help give the voters a sense that you are really interested in their communities and not just ideological about out-of-whack development and the autocratic policies of Mayor Bloomberg.
John Heyer: A fifth-generation Carroll Gardener, Heyer, at 27, is probably the closest thing to a Republican in the group. A devout Catholic, he opposes same-sex marriage but thinks that all couples deserve the rights you get from being married. His idea: The State should get out of the marriage business (an idea with merit but not a very realistic one at this juncture). Candid by nature and very articulate, hes been in politics since the age of 14 and obviously knows how to walk the walk with an admirable ability to connect with an audience about his belief that politics is about presence and constituency.
Unasked for advice: Refine your stump speech so that you quickly convey your commitment to the character of neighborhoods.
Brad Lander: His good nature and years of experience creating affordable housing Brooklyn bodes well for his future in the City Council. Just watching Lander interact with the other candidates at the forum, you can tell that hes a pleasant person to work with and a very fair guy. He combines deep smarts, great communication skills, and the ability to listen and learn from others.
Unsked for advice: You talk too fast and sometimes come across as a tad too sure of your front runner status.
Bob Zuckerman: The menchiest of the candidates, Bob isnt afraid to float out-of-the-box and sometimes wacky ideas. His proposed Zuck-Mobile, a mobile office made from a rehabbed Bookmobile, is taking on iconic stature in the campaign; many like his proposal to offer tax incentives to landlords to sell their storefronts as condos to local business owners.
Unasked for Advice: Your good natured, stand-up approach sometimes takes away from the seriousness of your out-of-the-box ideas.

The studys results suggest that there are distinct and separate hormonal risk factors associated with different subtypes of breast cancer. Amanda Phipps, a predoctoral research associate at the Fred Hutchinson Cancer Research Center in Seattle, and her colleagues conducted a study to better understand the specific risk factors for the subtypes of breast cancer, which are classified by expression of the estrogen receptor, the progesterone receptor, and the HER2 receptor. The investigators found that reproductive risk factors varied considerably by breast cancer subtype. Breastfeeding for 6 months or longer was associated with a lower risk of luminal cancer as well as triple-negative cancer, a type that can be particularly aggressive and difficult to treat. Both late age at menopause and use of estrogen plus progestin hormone therapy were associated with an increased risk of luminal disease. Finally, no differences in risks associated with number of children or the age when a woman first gave birth were observed by subtype. The study authors concluded that their results indicate that certain reproductive factors may have a greater impact on risk of certain molecular subtypes of disease compared to others. Despite having a high risk of early recurrence, the study indicates that triple-negative breast cancer patients who remain disease-free for eight years are unlikely to die of breast cancer and may be of their disease. Insulin resistance, hyperinsulinemia, and changes in the signaling of growth hormones and steroid hormones associated with diabetes may affect the risk of breast cancer. Despite many proposed potential pathways, the mechanisms underlying an association between diabetes and breast cancer risk remain unclear, particularly because the 2 diseases share several risk factors, including obesity, a sedentary lifestyle, and possibly intake of saturated fat and refined carbohydrates, that may confound this association. Although the metabolic syndrome is closely related to diabetes and embraces additional components that might influence breast cancer risk, the role of the metabolic syndrome in breast carcinogenesis has not been studied and thus remains unknown.

Ive started to lie and that is something I never do. But Im coming clean so that I wont have to lie again.
Even though I know when I get my results, I tell my family that I am not sure when I will get them. But I have come clean and told them that I will not tell anyone when I get the results because I feel too much pressure.
Imagine having everyone in your familys hopes resting on your shoulders. They are all so hopeful and anxious that the test results be okay that I feel more anxious about hearing the results. It is like I am not only waiting for the axe to fall on me, but to fall on my family too. And with the nature of Stage 4 Inflammatory Breast Cancer, the axe will fall at some point.
The other morning Jacquie was driving me to the hospital and she kept asking me So, what do you have today? Is it blood work and x-rays? I answer yes twice and then she asks me again. For fuck sake Jacquie, you know fucking well I am getting the results, why are you asking me. I didnt want to say because now I feel more pressure that you are going to be sitting for an hour worrying and now I have your fucking pressure too. Christ. I am crying and Jacquie starts crying. I feel better that I have passed on some of my angst to her.
The tears have given me some relief. Notice how I deflect my pain.
I am such a beast I have just been yelling at an angel. And in angel fashion, Jacquie apologizes to me.
My appointment is for 9 a.m. and I get brought in to the room. I change and put on a gown and then have to leave the room walking down the hallway with a bare back and my tail hanging down because I was placed in the wrong room. No big deal.
If you can believe it, I am feeling much more relaxed than I was the last time I came. Back then I almost had a panic attack.
In the new room I sit with my eyes closed thinking that if I keep them closed when the doctor talks to me I wont have to see the axe swing down. I often wonder how others cope with impending doom. What are other strategies people use while waiting for test results that may change their and their familys lives again? Over the last few years I have learned to live a lot in the now, but when the now is the waiting room and the possible axe it is a difficult now to live in. It is hard to be present when the present situation is extremely stressful.
In the end, sitting in the waiting room waiting to be called and then being brought to the examining room waiting to be examined I continue my self-talk. Fear is the mind killer. The results will be what they are. I am doing the best I can. Please God help me.
I tell myself that it will be okay. The bottom line is that I know I want it to be okay, but I know that it just might not be okay. I know that the possibility that it is not going to be okay and that the disease may not be responding to Femara any longer is real. I know that the cancer will spread even if I am tough and brave and believe. I know that the cancer will find its way around the treatment I am on and will bring me closer to my death; and I wonder if this is the time that will happen.
I have had to deal with the knowledge that since the word cancer came out of my doctors mouth that nothing will ever be okay again. Since that word and in the hours and days and months since; I knew it would never be clearly okay again.
I tell the nurse I want the results from my blood tests and tumor markers and she gets me all the forms. My hemoglobin is dropping and that explains my fatigue. My platelets are going up and that is a good thing as it means my bone marrow is doing better. My tumor markers are going up and that is not a good thing.
Dr. Grenier comes in within five minutes of me sitting in the new room and I am surprised because Im usually waiting there forever.
How are you feeling Renee? Oh I know you want your results right away from the scans. They are stable. That is good news.

Bone consists of a fibrous tissue called matrix on which calcium salts are laid down, as its outer part. This fibrous tissue is part of the outer membrane of the bone- the Periosteum.
Inside hollow bones, there is a soft tissue known as bone marrow. The bone marrow is found in a space called the Medullary cavity. The bone marrow can be either a fatty tissue only or a mixture of fat cells as well as blood generating cells. These blood generating cells in the bone marrow are responsible for the production of Erythrocytes, Leukocytes and Platelets, which are important components of blood.
The tissue known as cartilage, found at the two ends of a bone acts as a cushion between the bones.
The bone tissue or the osseous tissue contains two different kinds of cells. They are, Osteoblast and Osteoclast which are functionally known as the ‘bone forming cells’ and cells that dissolve or breakdown bone tissues, respectively.
The term ‘tissue’ is frequently mentioned above. A tissue can be defined simply, as a group of cells who have a common function. Therefore, it is obvious out of the many cells that form a bone, any can develop to a cancer.
There are several types of bone cancer, such as ‘metastatic cancer’ (which is a cancer that was spread from somewhere else from the bone, bone marrow related cancers that gives rise to ‘multiple myeloma’, leukemia also commonly known as blood cancer) and lymphomas (though it is often found in lymph nodes, there are occasions when it starts in the bone marrow
The main type of bone cancers are the bone tumors that starts in the bone, which are also known as primary bone tumors. Some of the bone tumors do lead to bone cancers; however, not all bone tumors are cancerous.
The following are the names of some cancerous primary bone tumors:
Osteosarcoma - (also known as Osteogenic sarcoma) one of the most common primary bone cancers that start in the bone cells.

It is hard to come to Williamstown without being struck by the majesty and seeming perfection of the place. Many of us go through our years here never looking past this “village beautiful” image. Rashmi Sinha, Ph.D., and colleagues at the National Cancer Institute, Rockville, Md., assessed the association between meat intake and risk of death among more than 500,000 individuals who were part of the National Institutes of Health-AARP Diet and Health Study. Participants, who were between 50 and 71 years old when the study began in 1995, provided demographic information and completed a food frequency questionnaire to estimate their intake of white, red and processed meats. They were then followed for 10 years through Social Security Administration Death Master File and National Death Index databases.
During the follow-up period, 47,976 men and 23,276 women died. The one-fifth of men and women who ate the most red meat (a median or midpoint of 62.5 grams per 1,000 calories per day) had a higher risk for overall death, death from heart disease and death from cancer than the one-fifth of men and women who ate the least red meat (a median of 9.8 grams per 1,000 calories per day), as did the one-fifth of men and women who ate the most vs. the least amount of processed meat (a median of 22.6 grams vs. 1.6 grams per 1,000 calories per day).
When comparing the one-fifth of participants who ate the most white meat to the one-fifth who ate the least white meat, those with high white meat intake had a slightly lower risk for total death, death from cancer and death from causes other than heart disease or cancer.
"For overall mortality, 11 percent of deaths in men and 16 percent of deaths in women could be prevented if people decreased their red meat consumption to the level of intake in the first quintile [one-fifth]. The impact on cardiovascular disease mortality was an 11 percent decrease in men and a 21 percent decrease in women if the red meat consumption was decreased to the amount consumed by individuals in the first quintile," the authors write. "For women eating processed meat at the first quintile level, the decrease in cardiovascular disease mortality was approximately 20 percent.

For those of you who cannot quite recall Guinier’s brief appearance on the national political stage, she was Bill Clinton’s choice for assistant attorney general for civil rights in 1993. I recall some initial excitement over the fact that a black woman had been nominated for such high position but her nomination quickly unraveled as her record came to light.
Guinier’s agenda of pure racial quotas and proportional racial representation in local elections was so profoundly radical that Sen. Carol Moseley-Braun, the only black woman in the U.S. Senate at the time, urged Clinton to withdraw the nomination, which he did.
Guinier is bringing the same incendiary brand of racial politics to higher education. In Guinier’s world, “when they talk about tests and they talk about metrics and they talk about looking at empirics, that is a way of making racism invisible.” Tell that to the millions of young black men and women who have successfully applied themselves to academic achievement and careers beyond it.
Guinier goes on to insult hard working students and diminish their academic success in explaining her rationale, saying, “the reason that I’m calling it racism is because it is a state of mind that is indifferent to the fact that these tests, whatever you think about them, are having a disparate impact on different populations and violating that first principal that talent is equally distributed among all groups.”
The audacity in this assertion is exceeded only in its staggering absurdity. Different people perform differently on standardized tests because people are, well, different. I may score high on tests involving history or language, but fear my expertise in higher mathematics is woefully lacking. The same holds true for those in vocational fields. An aptitude for auto mechanics doesn’t automatically translate into great skill in dental hygiene, welding, or any other trade. The facts are simple; talent is not, never has been and never will be “equally distributed among all groups,” as Guinier preposterously claims.
Guinier’s goal is simple and predicable. Agree with her and all is right with the world; disagree and you are a racist. The overwhelming bulk of data and research over many years prove the case for standardized testing for college admission, and Guinier turns to racial slander and political intimidation to advance a position that no serious person considers valid.
These tactics are reminiscent of how ideology was enforced in the Soviet Union. Russians who believed Soviet Communism to be anything other than an ideal social and political construct were deemed insane and exiled to the gulags. Similarly, Guinier seeks to marginalize those who disagree with her bizarre notions of educational excellence with accusations of racism.
The role of testing in college admissions deserves better than the race baiting duplicity we’re seeing from individuals like Guinier and political organizations like Fair Test, which shares Guinier’s desire to abolish standardized testing. They represent the new radical front in altering higher education in America and should be shunned or we risk the entire disassembly of the concepts of academic merit and excellence.
